HIV/AIDS Client Application
We make it easy to get the nutrition you need. To qualify for Open Arms’ meal-delivery service, you need to be:
- diagnosed with HIV/AIDS (if you have a life-threatening illness such as cancer, multiple sclerosis, ALS, COPD, ESRD or CHF, go here)
- either physically unable to shop or cook for yourself OR have a compromised nutritional status
- living in the Twin Cities metro area
Note: We do not qualify individuals based on income status.
How to Submit an application
To apply for our meal-delivery service, follow the instructions below. If you have any questions about the application process, please contact one of our Client Services representatives at 612-767-7333.

- 2. Fill Out Application
Fill out the PDF application form. Please note: A signature from your medical provider, case manager or social worker is required.
- 3. Submit application
Completed applications can be sent via email to [email protected], faxed to 612-872-0866, or mailed to our South Minneapolis building (c/o Client Services Department).
What Happens Next?
After we receive your completed application form, we will assess your eligibility and a member of our staff will contact you to determine your nutrition needs and to arrange meal deliveries that fit your schedule. If you are not eligible for services, our staff will assist you in finding other resources.
